Due to the fact that this stage has narrow KO zones it is hard to score a normal KO. As Sakurai said "Speaking frankly, this stage throws all the basic rules of Smash out the window!" You need to hit a "shellcreeper" (koopa) or a "sidestepper" (crab) from below (through the floor), pick it up, and throw it at someone; before you can get a KO.
It is unlocked by beating Event 19: Wario Bros.
