Actually it's quite naive and idealistic to believe that all the characters are of equal competitive playable strength. Dr. Mario's position on the tier list reflects improvements to the competitive viability of things like KO potential. His forward-air is a comboable and effective knockout move whereas you rarely see it applied with Mario as a spike due to the long windup time and subsequent ease to meteor-cancel on the part of the opponent. That in combination with the practical uses of pills over fireballs is enough to justify two spaces on the tier list, I would say.
