The Name Entry glitch is unusual for the amount of time it took to be discovered; it was discovered in 2008 on a German gaming website by Smashboards' Exchord92 and was posted to Smashboards in June 2009, a year after the release of Brawl and almost eight years after the release of Melee in 2001.

There are two ways to perform the Name Entry glitch, one using one controller, and the other using two controllers:

Method one
Method one
This method requires more precise timing, but is the only way to perform the glitch outside of Versus Mode. On the character select screen, the player must place the cursor over the name field of their character, and then press GCN and GCN simultaneously. Release GCN, but hold onto GCN. Because GCN has been pressed on the name field, the list of names scrolldown will appear. At the same time, because GCN is being held down, the game will go back to the menu in a few seconds. Within these few seconds, the player must scroll down to the "Name Entry" button and press GCN again. The timing of the second GCN press (to go to the Name Entry screen) and the effect of the held GCN button (to go back to the menu) must occur at the same time; if done correctly, the game will go to neither location but rather skip ahead (to the Stage Selection screen, if in Versus Mode). At this point, if the player can select a stage and have it load properly without the game freezing, they have successfully performed the Name Entry glitch.
To make this method easier, it is recommended to have only exactly one name present in the name dropdown. When this is the case, the cursor will automatically be on the "Name Entry" button upon pressing GCN for the first time in the name field box, so the player will not have to worry about quickly scrolling to the button and can focus solely on timing the second GCN press.

If the second GCN press is performed too early, the game will go to the Name Entry screen before the held GCN button has the chance to take them back to the menu. If the second GCN press is performed too late, the GCN button will send the player back to the menu before the GCN press has the chance to send them to the Name Entry screen. These facts can be used to help narrow down the timing of the second GCN press.
Place the cursor of player three's controller over the name field on the character select screen. On this controller, press A and B simultaneously. Release A, but hold onto B. Scroll down to the "Name Entry" button. If A is pressed again at the same moment that the game attempts to go back to the menu because of the B button, the game will skip to the Stage Selection screen. Select the stage. If it loads properly, the game has not frozen and the player will have performed Method One of the Name Entry glitch with success.

Method two
Method two
This method requires less precise timing, using a second controller to manually go back rather than relying on the timing window of the held GCN button. As a result, it can only be performed in Versus Mode.

With this method, the player must hover one controller's cursor over the "Back" button while the other controller opens the name field box and hovers over the "Name Entry" button. At this point, pressing GCN simultaneously on both controllers will cause the game to attempt to go back to the menu and go to the Name Entry screen at the same time, causing the game to skip ahead to the Stage Selection screen the same way as in the above method. (As above, if the game goes back to the menu, or goes to the Name Entry screen rather than the Stage Selection screen, then one of the two controllers pressed their GCN button too soon before the other.)
Using two controllers in VS. Mode, hover one cursor over the "Back" button and the other over the "Name Entry" button in the name field. Press A simultaneously on both controllers. The game should skip to the Stage Selection screen. If the stage the player selected loads up properly, they have performed Method Two of the Name Entry glitch with success. (This can be used with "Single Person Melee" by having another Player turn the person on the "Back" button to OFF instead of HMN or CPU)

Specific scenarios
Specific scenarios
Master Hand glitch
Master Hand glitch
The Master Hand glitch is the most popular of the four scenarios. To perform this subglitch, perform the Name Entry glitch while one or more characters in the match are human (HMN) players that do not have any character selected. These players will become Master Hand once the match starts.

While up to four Master Hands can be in the match, due to the way the game is programmed, only controller port 3 can actually control Master Hand. To minimize chances of the game freezing, use controller port 3 to select the stage on the Stage Selection screen. Once the match starts, the announcer will shout "Ready... Go!" as in a standard match, but the camera will pan over while Master Hand flies in laughing maniacally. If controller port 3 is able to control Master Hand, the player has successfully performed the Master Hand glitch.
The Master Hand glitch is the most popular of the four scenarios. It is performed by not selecting a character on one, some, or all of the four slots (note that although the player can use four Master Hands, he can only be controlled through Port 3). Perform the glitch normally (note that if Method Two is used, at least one character will not be Master Hand). Select the stage (using controller 3 to minimize chances of freezing). When the match starts, Master Hand will fly in laughing maniacally. However, the announcer still says "Ready.. GO!" like in a standard match. If the player is able to use Master Hand with controller 3, they have successfully performed the Master Hand glitch.

Black character glitch
Black Character glitch
This subglitch can be used to play as an all black character, similar to Dark Link in Event 18: Link's Adventure.

To perform optimally, set the match mode to Time, Team Battle, and Friendly Fire on. Next, select four of the same character and place them all on the same team, then perform the Name Entry glitch to start the match. Player 4 will appear as a black silhouette of their character. (The match mode can also be set to Coin or Bonus.)
This glitch can be used to play as an all black character.

Note that because all players in the match are on the same team and have no opponents, any CPU characters won't attack or move at all, except to recover or use items that they are attracted to (such as Poké Balls). If Friendly Fire is not turned on, no characters will be able to fight, unless bombs can be used. If the mode is set to Stock rather than Time, the match will end immediately (making it a case of the 0-Second Melee glitch below).
Set the mode to Team Battle. Next, select four of the same character and have them all on the same team. Perform the glitch normally. Player 4 will be a black silhouette of the character. Note that because all players are on one team, stock matches will end immediately, and CPU characters won't attack, as they have no opponent to fight (although they will recover, and use certain items, like Poké Balls). Friendly Fire can be turned on to simulate a normal time match.

0-Second Melee
0-Second Melee
This subglitch can be used to play a melee that lasts 0 seconds.

To perform, set the match mode to Stock with any number of stocks. Next, choose any character(s), but choose no opponent(s) (so if there are multiple characters, they must all be on the same team). Perform the Name Entry glitch to start the match. The match will end instantly once it starts, with the announcer shouting "Go!" and "Game!" at the same time. This also occurs in Stamina Melee.
This glitch can be used to play a melee that lasts 0 seconds.

Set the mode to stock, with any number of stocks. Next, choose a character, but choose no opponent. Use either method of the glitch. The melee should end the second the announcer says, "Go!"

Selecting PokeFloats in v1.00 will trigger the All-floats glitch.
Selecting PokeFloats in v1.00 will trigger the All Floats Glitch.

Single Person Melee
Single Person Melee
This subglitch allows the player to play on the stage alone, with no opponents or other players.

To perform, set the match mode to Time, Coin, or Bonus, then choose a single character and no opponents or other players. Perform the Name Entry glitch to start the match. When performed correctly, the player will be fighting on the stage alone, with the camera somewhat zoomed in on them as normal.
This glitch allows the player to play alone, with no opponents.

Note that this subglitch and the Master Hand glitch can be done at the same time, by not choosing a character. However, the game will freeze if Master Hand uses the finger walk attack (GCN+GCN), as he will have no opponent to target. It is unknown why Master Hand's other attacks do not similarly crash.
Set the mode to time, bonus, or coin. Next, choose a character, but choose no opponent. Use either method of the Name Entry glitch. If performed correctly, the player should be fighting alone. Note that this and the Master Hand glitch can be done at the same time if the player didn't choose a character; however, if the player attacks, the game will freeze as he has nothing to home in on.

Explanation
Explanation
As mentioned above, the Name Entry glitch works by forcing the game to attempt to go to two different menus at once, causing it to end up going to neither but rather skipping ahead to the Stage Selection screen (or the equivalent screen after the character selection screen, outside of Versus Mode). The effect of this is to allow the player to force a match to start, even under conditions that the game would not allow (namely, having human players who haven't selected a character yet, or having no opponents selected in the match).

The Master Hand glitch works because all characters in the game have an ID number (for example, Mario has an ID of 1), and Master Hand has the default ID number of zero. However, zero is also used as a placeholder on the character select screen for human players who have not picked a character yet, and the game is programmed to reject this ID number and not allow the match to start if any players have invalid character IDs. The Name Entry glitch effectively forces the match to start regardless, thus causing any unselected players to have Master Hand (character ID zero) loaded as their character.
The Name Entry glitch forces the game to receive two inputs at once: Back and Name Entry. In its confusion, it advances to the Stage Selection screen; the equivalent of the Name Entry screen function outside of the character selection screen. The reason that the Master Hand Glitch works is because all characters in the game are assigned an ID number. The game is programmed not to let the players progress to the Stage Select menu if not all of the players have valid ID numbers. The default number, zero, is also Master Hand's number and is not supposed to be accepted by the game. However, when the glitch is performed, it forces the game to the Stage Select screen, leaving the players' numbers as zero, thus loading Master Hand as their character.

The Black Character glitch is a side effect of the game's programming when two or more of the same character are on the same team. Characters on the same team must have the same alternate costume, so to differentiate between multiples of the same character, the game applies sub-colors to the costume. In a normal match, up to three players can be on the same team, so if all choose the same character, Player 1 will have no sub-color, Player 2 will have a lighter sub-color, and Player 3 will have a darker sub-color. It is normally not possible for four players to be on the same team (because then they will have no opponents), but by bypassing this requirement with the Name Entry glitch, the game is forced to load the fourth sub-color for Player 4, which happens to be all black. (These sub-colors can be viewed in this order in the Debug menu, which also reveals that there exists a fifth sub-color which is all grey. This fifth sub-color is inaccessible outside of the debug menu.)
The All Black Character Glitch works, because when two or more of the same character are on the same team, those same characters have to be the same color. To help identify who's who, the game has sub-colors it throws on these characters. P1 has no sub-color, P2 is lighter, And P3 is darker. Normally, there aren't supposed to be 4 people on one team, but in this case there is, so the game loads the next sub-color, which happens to be all black (there is also an "all grey" sub-color, only available through the debug menu).
